Naval Health Clinic Hawaii

E komo mai (welcome) to Naval Health Clinic Hawaii, where your health and readiness is the focus. NHCH is composed of two large clinics, Makalapa at Joint Base Pearl Harbor-Hickam and Kaneohe Bay at Marine Corps Base Hawaii. Other smaller clinics are at Camp H.M. Smith, Naval Communications and Naval Computer and Telecommunications Area Master Station, Pacific, Wahiawa and Pacific Missile Range Facility, Barking Sands, Kauai. Welcoming Center

Your introduction to care at NHCH begins at the Welcoming Center, where TRICARE Prime beneficiaries begin a one-stop check-in process to enroll to NHCH. The staff will assist you with TRICARE enrollment, medical record updating, primary care manager assignment and initial appointments for any care. Active-duty members will also update their Periodic Health Assessment requirements, part of the Individual Medical Readiness process. UnitedHealthcare Military & Veterans representatives and Health Benefits Advisors are available to answer TRICARE and enrollment questions. During the check-in process, information about several resources, including TRICARE Online, the Exceptional Family Member Program and advance directives, will be provided. Bring ID cards, Social Security numbers, private health insurance cards, medical records, dental records (if active duty) and immunization records when you check in. NHCH will request that your medical record (civilian or military) be sent from your previous provider. A Request for Medical/Dental Records, DD Form 877, will facilitate this process if completed prior to moving to Hawaii. This form is available from your current military treatment facility medical records department. A copy of your civilian records will also be requested.

Welcoming Center Makalapa Clinic is at the Makalapa Annex. Call 808-473-2444, ext. 501. Hours are 7:30 a.m. to 4:30 p.m. Monday through Friday. The TRICARE Service Center is in the Welcome Center and is open 7:30 a.m. to 4:30 p.m. Monday through Friday.

Appointments and After-Hours Care

For life-threatening conditions, dial 911 or go to the nearest emergency room.

To make a medical appointment online, go to www.tricareonline.com and click on “Appointment Center” (available 24/7).

NHCH After-hours Provider Advice Line: If you need to speak with a medical provider when the clinics are closed, call 808-473-0247, ext. 3 and you will be connected to an answering service who will contact the on-call provider. The provider will be able to help you decide how to treat an illness after hours or if you should go to the emergency room.

Makalapa Clinic

The Makalapa Clinic is on Joint Base Pearl Harbor-Hickam just inside the Makalapa Gate, Building 1407, 1253 Makalapa Road. Hours are 7:30 a.m. to 4:30 p.m. Monday through Friday. It is closed weekends and federal holidays. Services provided include family medicine, pediatrics, dermatology (active duty only), gynecology, immunizations, laboratory, optometry, pharmacy, preventive medicine, radiology and comprehensive dental care (active duty only), physical therapy (active duty only), sports medicine (active duty only) and deployment health. Immunizations Clinic

Yellow Fever administration: 7:30 a.m. Thursday (walk-in only with a minimum of three people)

Allergy shots: 1 to 3 p.m. Monday through Wednesday and 7:30 to 9 a.m. Friday (walk-in only)

To allow time to complete paperwork, beneficiaries are requested to arrive at least 30 minutes before closing time.

Radiology

X-ray, ultrasound and mammogram services are provided. Women over the age of 40 can request their own mammogram without referral as long as they provide their PCM’s name. Mammogram appointments are available 8 a.m. to 2:30 p.m. weekdays. CT, MRI and specialized X-ray services are provided at Tripler Army Medical Center.

Shipyard Clinic (Occupational Health)

The Shipyard Clinic is on Joint Base Pearl Harbor-Hickam in NHCH Headquarters, Building 1750, 480 Central Ave. The clinic provides occupational health, audiology, preventive medicine, industrial hygiene, radiation health, laboratory, X-ray and immunization services.

Exceptional Family Member Program

The Exceptional Family Member Program identifies and documents the special medical, mental health or educational needs of family members to ensure those needs are considered during the assignment process. Enrollment in EFMP is mandatory for all family members of active-duty service members with needs requiring specialized services. The NHCH EFMP coordinator is at the Joint Patient Liaison Office, Tripler Army Medical Center, 1 Jarrett White Road, Seventh Floor, Oceanside. Call 808-433-9644. Hours: 7:30 a.m. to 4 p.m. Monday through Friday.

Dental Services

Makalapa Clinic Dental Care

Active-duty personnel dental services are in the Makalapa Clinic on Joint Base Pearl Harbor-Hickam just inside the Makalapa Gate, Building 1407, 1253 Makalapa Road. For dental appointments, call 808-473-1880, ext. 93204. Hours are 7 a.m. to 4 p.m. Monday through Friday.

The dental clinic also provides after-hours emergency care. For dental emergencies after hours and on weekends, call 808-864-4705.

TRICARE Dental Program

Information on the TRICARE Dental Program is available at www.metlife.com, or by calling 800-638-5433. The TRICARE Family Member Dental Plan is a dental insurance plan for family members of active-duty military to receive dental care with civilian dentists. All family member dental care is provided within the civilian community. To ensure your family members’ dental needs are met, we encourage active-duty military to enroll family members into the TRICARE Family Member Dental Plan. This will avoid costly dental bills.
