Welcome to Grand Forks Air Force Base and the 319th Medical Group!

We are a robust outpatient medical clinic capable of providing full service family medicine, flight medicine, pediatrics, women’s health, behavioral health services, dental care, optometry, physical therapy, immunizations, and an ambulance service. Our ancillary services include pharmacy, radiology and laboratory. In addition, we run the health and wellness center, vet services, public health and bioenvironmental engineering sections.

Altru Health System and Aurora Medical Center in Grand Forks are the Medical Group’s primary referral facilities for medical specialties that are beyond our scope of care. Your Primary Care Manager (PCM) will refer you for specialized medical care as required.

Our patients are our top priority and our emphasis is on customer satisfaction. If you have questions or suggestions, please address them to any of our staff. All of our staff are trained as Patient Advocate’s and are ready to assist you in solving any challenges you may face. Continuous service improvements are important to us so your suggestions are welcomed and encouraged. We will make every attempt within our resources to provide you with the best possible medical care.

Medical Clinic

The Grand Forks AFB Clinic, located at 1599 J Street (Building 109), provides routine and acute care Monday through Friday. However, there is no emergency room at Grand Forks AFB. For medical emergencies, call 911 or proceed directly to your nearest emergency room. The Clinic does provide 24-hour on-base ambulance service, if needed, to the Altru Hospital emergency room in Grand Forks. For out-of-area emergency and urgent medical care, contact the TRICARE office for information.

Active duty personnel have first priority at the Clinic. Retirees and family members enrolled in TRICARE Prime are guaranteed access to the Clinic. Those beneficiaries not enrolled in TRICARE Prime will only be seen on a “Space Available” basis.

The Grand Forks AFB Clinic serves a local patient population of more than 4,529 active duty and retired members and their families. Clinic hours of operation are from 7 a.m. to 4:30 p.m. Monday, and Tuesday through Friday from 8:30 a.m. to 4:30 p.m. It is closed weekends, Holidays, and Goal Days. Immunizations are given weekdays during the Clinic’s normal operating hours.

Special needs families--or Q-coded families--will have completed a facility determination before coming to Grand Forks AFB. Service and support groups will be referred by their specialty doctor or their primary care manager.

Making an Appointment: All appointments (Acute, Routine, and Wellness) will be sought through your Primary Care Manager (PCM) whether you are enrolled in Family Medicine, Flight Medicine, or Pediatrics. If you need more specialized care or procedures, your PCM will refer you to the appropriate specialist for the required care. You can obtain an appointment with your PCM by calling the central appointment desk at 747-5601 or 1-866-747-5601 or by utilizing TRICARE online services (Military Treatment Facility enrolled Prime and Plus only).

Emergency Care: A condition that poses a threat to life, limb, eyesight, or a sudden, extremely painful condition which requires immediate treatment to alleviate suffering can be obtained by either calling 911 for ambulance services or by going to the nearest emergency room (Grand Forks, Altru is the only emergency room in the local area. If you are unsure whether your situation is an emergency, you may call 747-5601 or 1-866-747-5601 to speak to your PCM team. See below if emergency care is required after hours.

After Hours Care: If you or your family members become sick after normal duty hours and it is not an emergency (definition listed above), you should contact the on-call provider (PCM) to obtain medical advice or authorization to seek civilian medical care. Call 747-5601 or-1-866-747-5601 and our answering service will request information from you to forward on to the PCM on call. The PCM will then contact you for further information and recommendations.

**Remember, except for emergency care, you must contact your PCM prior to receiving treatment. If AD family members and retirees seek medical care at a civilian facility without proper authorization, they risk Point of Service (POS) charges, which are a $300 for individual and $600 for family deductible, and a 50% cost share of the bill with no catastrophic cap. If AD members seek care without proper authorization, they may be responsible for the entire bill.

Dental Clinic

The Dental Clinic is open Mondays from 7 a.m. to 4:30 p.m. and Tuesday through Friday from 8:30 a.m. to 4 p.m.

Comprehensive dental care is available to all assigned active duty members and reservists on active duty. Dental sick call is held daily by appointment.

Routine care for other classes of beneficiaries is limited and prioritized below:
• Family members of active duty members, on a stand-by space available basis only;
• Retired personnel on a stand-by space available basis only;
• Family members of retired personnel on stand-by basis only;
• Emergency dental service to relieve severe discomfort, severe bleeding, or acute infection is available for all eligible beneficiaries.
Family members of active duty personnel are highly encouraged to enroll in the United Concordia TRICARE Family Member Dental Plan to obtain dental care from civilian dentist. Retired personnel are highly encouraged to enroll in the Delta Select USA TRICARE Retiree Dental Program to obtain dental care from civilian dentists.

TRICARE Family Dental Plan Customer Service Inquiries can be made by calling 1-800-866-8499. TRICARE Family Dental Plan Eligibility questions can be addressed through the United Concordia website.

The Delta Select USA TRICARE Retiree Dental Program toll-free number 1-888-838-8737. Answers to questions concerning enrollment, benefits are provided as well as a list of network dentists. For more information, you may also visit the Delta Dental website.

Pharmacy

The Base Pharmacy is open for outpatient prescription services. This service is available to all eligible beneficiaries. All items dispensed from the Pharmacy require a prescription. We honor military prescriptions and VA prescriptions as well as prescriptions from civilian providers for all medications that we stock. A 90-day supply is allowed for most medications. Civilian prescriptions are subject to the same quantity and refill restrictions as military prescriptions written at this facility. Check with the pharmacy for restrictions.

A valid military ID card belonging to the person for whom the medication is prescribed must be presented (this is not necessary for family members under the age of ten). An up-to-date Yellow Card (Third Party Collections) is required when picking up for anyone who is not currently Active Duty. Pharmacy personnel are available for counseling and guidance on all medications, including over the counter items, whether dispensed here or not. Please feel free to call the Pharmacy at 747-5382.

Refills are available through an automated telephone refill system. Call 747-3025 to access the system 24 hours a day. The same system allows the user to receive medication information and dispensing status for a specific prescription. Pharmacy personnel are available for counseling and guidance on all medications, including over the counter items, whether dispensed here or not. Please feel free to call the Pharmacy at 747-5382.

Family Medicine Clinic

The Family Medicine Clinic provides primary care for all ages and is staffed by a team of health care professionals to include physicians, physician assistants, nurse practitioners, registered nurses, and medical technicians.

All Grand Forks AFB personnel and their authorized beneficiaries can be empanelled with a healthcare team and Primary Care Manager (PCM) at Grand Forks AFB. Some health care must be referred to the civilian community. Referral to civilian facilities can be provided by the PCM if the care cannot be provided in the Family Medicine Clinic. The care must be deemed necessary by the PCM (i.e. surgery, orthopedics, OB, etc) and TRICARE before civilian providers can be seen. The clinic does not provide a weekend provider for the Grand Forks AFB population for any acute needs.

Health & Wellness Center

Located within the Fitness and Sports Center (Bldg. 308), the HAWC offers a variety of opportunities designed to enhance people’s quality of life and sense of well-being.

The HAWC is dedicated to help people adopt a healthy lifestyle for life. Healthy choices can help prevent the top four causes of death in the United States. The core health promotion programs include classes and counseling on nutrition, fitness, tobacco cessation and stress management. Additional programs include monthly national health observances, prenatal nutrition and fitness classes, Body Gem (where individual metabolism is calculated with computerized food log), Healthy Heart as well as classes for individuals interested in learning about proper nutrition for weight loss/control or management of blood pressure and cholesterol.

Our Relaxation Room offers a quiet place to relax where you can enjoy a variety of soothing or inspirational CDs or watch a calming video while sitting in a massage chair. The HAWC also offers literature on a variety of health promotion and disease prevention topics.

The Allergy & Immunization Clinic

The Allergy/Immunization Clinic provides allergy testing, injections, and immunizations for all ages. Allergy injections are given on Monday, Wednesday and Friday from 7:30 a.m. to 11 a.m. Allergy skin testing is scheduled through the Allergy/Immunization Clinic. A 30-minute wait is required after allergy injections. Routine immunizations are given Monday through Friday on a walk in basis. It is highly recommended for dependents to bring their shot records when visiting the clinic.

Laboratory

The Clinical Laboratory is nationally accredited by the College of American Pathologists. The lab processes approximately 3,500 specimens per month. Those specimens are processed and tested in the following departments of the laboratory: hematology, urinalysis, microbiology, chemistry and immunology. Specimens not processed in-house are shipped to another certified laboratory. An on-call technician is available on holidays and weekends.

A health care provider must request all laboratory testing using CHCS or a handwritten lab form. The laboratory staff is not allowed to give test results to patients. Your primary care or network physician will get back with you regarding the results of any abnormal laboratory tests. If you are concerned about your results, contact the requesting physician’s clinic.

Optometry

Please call the Optometry Clinic at 747-5515 for a routine eye exam appointment. The Optometry Clinic sees non-Active Duty patients on a space-available basis.

Optometry provides routine vision exams for people needing spectacle prescriptions. The clinic also diagnoses and treats common eye diseases and can refer cases requiring more complex care to other medical specialties. Other miscellaneous services include: driver’s license exams, preschool screenings, retiree screenings, color vision testing, and intra-ocular pressure screening.

The Optometry Clinic is closed during lunch hour from 11:30 a.m. to 12:45 p.m.

PATIENT ADVOCATE
Each clinic or section has a Patient Advocate who is available to resolve patient concerns in that particular clinic or section. If you are unable to work out a problem at that level, feel free to contact our Clinic Patient Advocate. If you have any concerns regarding your health care at the 319th Medical Group, please contact the Clinic Patient Advocate.

PEDIATRIC CLINIC
The Pediatric Clinic is a specialized area of health care delivery focused on providing care for children from newborn to 18 years of age. Acute appointments are provided daily. A legal guardian must accompany all patients during their appointment.

The pediatrician provides a wide range of services, to include Well-baby exams, initial assessments and follow-ups for acute and chronic illnesses. Patients who require services beyond the scope of this facility will be referred to a treatment center capable of providing the care needed. Routine, Wellness, Follow-up and Acute appointments are available by calling the 319 MDG appointment desk.

Well-baby exams are designed for infants from birth to 24 months of age; these appointments provide a complete preventive health care program. Services include complete physical examination, developmental evaluation, and instruction in nutrition, safety and other anticipatory guidance. Parents are responsible for scheduling all appointments at the designated intervals as follows: AGE: 3-day, 2 weeks, 2, 4, 6, 9, 12, 15, 18, and 24 months, and yearly up to age 6 then every other year except for requirements on CDC and physical activities.

PHYSICAL THERAPY
Patients are seen in the Physical Therapy clinic by referral from your Primary Care Manager. Please contact the 319th MDG appointment desk at 747-5601 to schedule appointments.

The Physical Therapy Clinic is staffed by a Physical Therapist and two Physical Therapy Technicians. Physical Therapy services include, but are not limited to, evaluation and treatment of primarily orthopedic injuries/diagnoses. Physical Therapy treatments include manual therapy, therapeutic exercise techniques, functional rehabilitation programs, limited gait training and various pain relieving modalities. The goals of the Physical Therapy Clinic are pain reduction, restoration of function following an injury, and patient education. Through examination and individualized treatment programs, the Physical Therapy Clinic can treat existing problems and provide health care for patients with a variety of needs.

PUBLIC HEALTH
Public Health is responsible for educating and assisting military members and their family members on preventive medicine, disease control and medical intelligence.

RADIOLOGY
Routine x-rays for all eligible beneficiaries require a written/electronic referral from a health care provider. A technician is on-call weekends and holidays to support the Acute Care Clinic. There is no radiologist assigned to the clinic. All x-ray images produced at 319th MDG are transmitted to MacDill AFB radiologists for interpretation. Please make all requests for x-ray images to be taken to a downtown health care provider two days prior to appointment.

TRICARE Prime patients age 40 to 49 can refer themselves to Altru Clinic for a routine screening mammogram every two years and patients age 50 and over can refer themselves every year. If you do not meet the above criteria, you will need a consult from a physician to get a mammogram.

TRICARE
All Active Duty personnel are automatically enrolled and must remain in TRICARE Prime. For all other eligible beneficiaries, the TRICARE program offers three options: Prime, Standard, and Extra.

TRICARE Prime is the premium health care option offered by military hospitals and clinics, supported by contracted civilian health care companies. To assure that adequate professional staffing and resources are available, an enrollment process is necessary. Enrollees selecting this option choose a military Primary Care Manager (PCM) to provide or arrange for their health care needs. The TRICARE Prime option offers the normal primary health service provided by a military hospital or clinic, plus additional wellness and preventive care services. 

TRICARE Standard is a fee-for-service option that gives the greatest choice of civilian physicians, but this comes at the greatest cost. The cost of having this choice includes an initial amount, called a deductible, plus a percentage of subsequent charges, called cost shares

TRICARE EXTRA is similar to TRICARE Standard but offers discounts to patients when they use TRICARE network providers. This option allows beneficiaries to receive their care from civilian network providers at a reduced cost compared to TRICARE Standard. There are no claim forms to file — just pay your reduced cost share after satisfying the deductible. You may use a combination of the TRICARE Extra and Standard programs at any time, depending on whether you choose physicians inside or outside the network. There is no separate enrollment for either of these programs. Contact your Regional TRICARE Contractor to locate a network provider, hospital or pharmacy.

TRICARE Service Center is available in the Clinic on a walk-in bases Monday through Friday from 7:30 a.m. to 4:30 p.m. The TSC is staffed with health care professionals who will assist you with choosing a TRICARE health plan, answering your benefits questions, making appointments for services at the 319th Medical Group, and providing claim information.
