How to Enroll for Medical Care at NHCH

Medical care at NHCH begins with active-duty members, family members and retired PRIME beneficiaries enrolling to NHCH for care by visiting www.tricare.mil. Beneficiaries can enroll in TRICARE or change their Primary Care Manager (PCM)/Military Treatment Facility. If you need personal assistance, please call the UnitedHealthcare Military & Veterans (TRICARE Regional Contractor for the West Region) toll-free call center at 877-988-9378. “Your care — Your choice!” Eligible non-active duty beneficiaries may enroll into TRICARE Prime at any Hawaii Military Treatment Facility. For more information, call UnitedHealthcare Military & Veterans toll-free call center at 877-988-9378 or visit www.uhcmilitarywest.com.

Exceptional Family Member Program (EFMP): The Exceptional Family Member Program identifies and documents the special medical, mental health and/or educational needs of family members to ensure those needs are considered during the assignment process. Enrollment in EFMP is mandatory for all family members of active-duty service members with needs requiring specialized services. The NHCH EFMP Coordinator is at the Joint Patient Liaison Office, Tripler Army Medical Center, Wing B, Room 1B101; open 7:30 a.m. to 4 p.m. weekdays. Telephone: 808-433-9644.

TRICARE Dental Program: TRICARE Dental Program information is available directly from MetLife at www.metlife.com or by calling 800-MET-LIFE (638-5433). The TRICARE Family Member Dental Plan is a dental insurance plan for family members of active-duty military to receive dental care with civilian dentists. All family member dental care is provided within the civilian community. To ensure your family members’ dental needs are met, we encourage active-duty military to enroll them into the TRICARE Family Member Dental Plan (TFMDP). This will avoid costly dental bills.

What’s an Emergency? TRICARE defines an emergency as a medical, maternity or psychiatric condition that would lead a “prudent layperson” to believe that a serious medical condition exists, or that the absence of immediate medical attention would result in a threat to life, limb or sight, and requires immediate medical treatment or which has painful symptoms requiring immediate attention to relieve suffering. For emergencies, such as profuse bleeding, unconsciousness, chest pain and obviously broken bones, dial 911, or go to Tripler Army Medical Center or the nearest emergency department.
