Primary Care Manager Services

Reynolds Army Community Hospital has transitioned our PCM services to Army Medical Home (AMH) teams by using the concept of Patient-Centered Medical Home (PCMH). PCMH is a new model of healthcare being adopted by leading healthcare systems across the country. The patient-centered medical home is a team-based model of care that provides comprehensive primary care for the entire family. In this model, a team of health professionals, coordinated by a physician, works collaboratively to provide high levels of care, access and communication with its patients. The physician-led team is responsible for providing all the patient’s healthcare needs and, when needed, arranges for appropriate care with other qualified physicians. At its heart, the PCMH is healthcare the way it should be – easily accessible, patient centered, healthcare provider team-based, and quality-focused. Your assigned PCM will be located either in the outpatient clinic mall within the hospital or at the Frontier Medical Home located at 5404 SW Lee Blvd. in west Lawton. 

Appointments for all home teams can be made online 24 hours per day online or by calling (580) 558-2000, Monday through Friday, 7 a.m. to 4 p.m. 

Family Medical Homes
Our Primary Care Managers participate within Teams and Family Medical Homes. At the main hospital the teams operate Monday through Friday, 7:30 a.m. to 4:30 p.m., and include Loyalty, Respect, Honor, Integrity and Courage. These teams are all located on the first floor, north end of Reynolds and are designed for patients of all ages. 

Frontier Medical Home
The Frontier Medical Home (FMH) includes teams Blue and Gold and is located at 5404 SW Lee Blvd., in west Lawton, adjacent to Southwestern Medical Center. It is well positioned where the majority of Army Families assigned to Fort Sill live. The FMH is part of the Army’s Community Based Primary Care Clinic (CBPCC) initiative to bring Army Medicine into the communities in which Army Families reside. Hours of operation are Monday through Friday, 8 a.m. to 5 p.m. The FMH also has a pharmacy with initial and refill capability for primary care prescriptions, and a limited laboratory. Internal Medicine
The AMH Internal Medicine Team is designed for patients 18 and older. Internal Medicine also takes referrals from other physicians to evaluate patients and/or perform selected procedures such as EKGs, treadmills, and holter monitors. The clinic is located on the 2nd floor of the outpatient mall. 

Fires Center
The AMH Fires Center includes 2 teams: Diamond and Leaders. Soldiers can access care after unit triage by booking online at or by calling (580) 558-2000. The clinic is located on the 1st floor of the outpatient mall. 

The AMH Pediatrics is designed for patients from birth to 16 years of age. Pediatrics also accepts referrals from other physicians to evaluate patients and/or perform selected procedures. Pediatrics is located on the 1st floor near the west end of the outpatient mall.
