TRIPLER Army Community Hospital Community
Enrolling to Naval Health Clinic Hawaii
Medical care at NHCH begins with the TRICARE Service Centers (TSC), where active-duty members, family members and retired beneficiaries begin their check-in process to enroll to NHCH for care. Staff will assist with TRICARE enrollment, primary care manager (PCM) assignment, registration into CHCS/AHLTA and Medical Record updates. Active-duty members will also be directed to complete the Periodic Health Assessment (PHA), part of the Individual Medical Readiness process. Staff to include health benefits advisors are available to answer TRICARE and enrollment questions. During the check-in process, information about several resources are provided, including TRICARE Dental, TRICARE Online, Exceptional Family Member Program and Advance Directives.
Please bring ID cards, Social Security numbers (for all enrolling), medical records, dental records (active duty only) and immunization records when you check in. A Request for Medical/Dental Records, DD Form 877, will facilitate this process if completed prior to moving to Hawaii. This form is available from your current MTF Medical Records Department. If you do not have your medical record, the Medical Records Department will request it to be sent from your last provider. You can still complete the check-in process. If you have never had a military health care system medical record, we will make one for you. For further information about the check-in process, please call the Health Benefits Advisor.
