Allergy and Immunization 433-8129
Allergy Shots: 2 to 3:30 p.m. Tuesday, 2:30 to 3:30 p.m. Thursday (during this time we do not give immunizations).
Immunizations: 7:30 to 11:30 a.m. and 1 to 3 p.m. Monday through Thursday (excluding the times allergy shots are given – see above).
Thursday Immunizations:
Administered 7:30 to 11:30 a.m.
Tuberculosis (TB) Tests: Done daily during immunization hours except Thursday.
Patient Assistance Line (PAL) USAHC-SB

Hours: 7:30 a.m. to noon and 1 to 4 p.m. Monday, Tuesday, Thursday and Friday, and 9 a.m. to 2:30 p.m. Wednesday.
You may call the PAL to leave a message for your PCM, request a medication refill, ask about lab and X-ray results, or talk to a nurse. Messages will be returned within 72 hours.

Walk-in Hours for Immunizations ONLY:
8 to 11 a.m. and 1 to 3 p.m. PPD is not given on Thursday or before long weekends/holidays.
Services Provided:
- Walk-in immunizations for empanelled patients.
- Well baby exams.
- Same-day acute and follow-up appointments.
- School/sports physical.
- Patient education.
Additional Information for Family Primary Care
Late Policy: You must check in by your appointment time. If you check in after your appointment time you are considered late. Late patients are seen on a space-available basis. Patients checking in 10 or more minutes after their appointed time are considered a “no show.”
- Allow extra time for parking and walking to the clinic.
- The Armed Services YMCA provides free hourly child care (six weeks to 12 years of age) in Bldg. 680 during certain business hours. Call 624-5645 to arrange child care with this service. Hourly care is also available at the Child Development Center.
- If your physician recommends that you make a follow-up appointment, please make one immediately. If you wait, you may not be able to get an appointment with your PCM.
Minors: Patients under the age of 18 must be accompanied by a parent or legal guardian. If another adult will be accompanying the child, please provide the adult with a medical power of attorney authorizing them to consent to care for your child. By law, one exception exists: Patients aged 14 to 18 can choose to be seen without a parent for OB, family planning and sexually transmitted disease reasons.
- Do not bring children to Well Woman or new OB appointments at Family Medicine. A responsible family member age 12 or older or a friend can come with you and watch your children in a waiting area during your exam. See YMCA information listed above.
