Naval Hospital Lemoore
Naval Hospital Lemoore is a 150,000-square-foot, 16-bed community hospital located on NAS Lemoore in the heart of central California. Fully accredited by the Joint Commission and having earned their “Gold Seal of Approval,” Naval Hospital Lemoore is proudly committed to providing you with comprehensive, patient-centered health care. Naval Hospital Lemoore delivers primary care using Medical Home Port (MHP) teams. Your MHP health care team will consist of your primary care manager (PCM), a registered nurse (RN), a licensed vocational nurse (LVN) and one or more hospital corpsmen. Your team will partner with you to provide the best possible evidence-based care to fulfill your health care needs.
The Medical Home Port hours are 7:30 a.m. to 4:30 p.m. Monday through Friday. The practice is closed weekends and all federal holidays. If you need medical advice during office hours, you may contact your MHP team by telephone, in person at your team’s reception desk, or by sending a secure electronic message using RelayHealth. If you would like to talk with a provider after hours, your Medical Home Port has a provider available to talk with you by phone when the practice is closed. To reach a provider, please call 559-998-4481 and ask to speak to the Medical Home Port on-call provider.
Medical appointments can be made with Central Appointments at 559-998-0889, 6:30 a.m. to 4 p.m. Monday through Friday. If you are a patient assigned to a Medical Home Port Team, please register at www.relayhealth.com for secure online communication with your provider team. Your Medical Home Port Team will do all they can to make same-day appointments available for you and your family.
HORNET HEALTH CLINIC (HHC)
The Hornet Health Clinic is available for active-duty Sailors who work on the operations side of NAS Lemoore. The HHC provides primary care, aviation medicine, chiropractic care, occupational health, preventive medicine, industrial hygiene and optometry. Ancillary services include: pharmacy, radiology, laboratory and immunizations.
Primary Care and Aviation Medicine clinics provide comprehensive outpatient primary medical care to active-duty personnel only. The Chiropractic Clinic is a self-referral clinic for active-duty personnel only, with no referral needed. The Occupational Health Department provides health screening and physicals, and audiometric testing and monitoring. Preventive Medicine provides medical surveillance and counseling on a healthy lifestyle. Industrial Hygiene evaluates environmental stresses in the workplace.

Naval Hospital Lemoore does not have an emergency room (ER). For emergencies involving potential loss of life, limb or eyesight, call 911 or have someone take you to the nearest emergency room. If you are admitted into a civilian hospital, you or a family member are required to contact our hospital or notify UnitedHealthcare Military & Veterans at 877-988-9378 within 24 hours, or as soon as medically reasonable. As your health care provider, we want to work with you and your family to make certain you receive any necessary follow-up care and that you make a smooth transition back to your home.
Naval Hospital Lemoore keeps and maintains outpatient records. To avoid lost medical information, please request a copy of civilian medical records be sent to Naval Hospital Lemoore following any care received outside of the military health system. We can also copy portions of your record if you need medical care at another location. Whenever possible, please provide a week’s notice to properly process copies. Ideally, a 30-day notice is preferred prior to a PCS move.
Naval Hospital Lemoore offers several options for obtaining your prescriptions:
- The hospital pharmacy is open 7:30 a.m. to 4:30 p.m. Monday through Friday. It is closed weekends and holidays.
- Prescriptions written by civilian providers are filled 7:30 a.m. and 4:30 p.m. weekdays. Your prescription can be faxed directly from your doctor’s office to 559-998-4529.
- In addition to obtaining your prescriptions at Naval Hospital Lemoore, for your convenience, we encourage the use of TRICARE Pharmacy Home Delivery. For additional information on TRICARE Pharmacy Home Delivery, please call the Customer Care Center at 877-363-1303 or visit www.express-scripts.com/tricare. Mail order forms are also available at the hospital pharmacy. For any questions, please call 559-998-4499.
The Wellness Center offers educational programs on nutrition, weight management, meal planning, diabetes meal planning, cholesterol, fitness assessments and tobacco cessation. Most appointments can be booked through Central Appointments at 559-998-0889.
Comprehensive dental care is provided for active-duty members assigned to NAS Lemoore at the Mainside Dental Clinic and the Hornet Health Dental Clinic. Specialty care available on-site includes oral surgery, periodontics and prosthodontics. Family members and retirees who are members of theTRICARE dental insurance plan are urged to contact their civilian dental provider for treatment. In addition to routine care, emergency dental work is also covered under this policy.
Hours of operation are 7 a.m. to 4 p.m. Monday through Friday. Appointments may be scheduled by calling the Mainside Dental Clinic at 559-998-4220/4219 or the Hornet Health Dental Clinic at 559-998-4300. Sick call is held at either clinic 7:30 to 10 a.m. Monday through Friday; and 1 to 2 p.m. Monday through Thursday. For emergencies after clinic hours, contact the Immediate Care Clinic at 559-998-4435.
REHABILITATION PROGRAM (SARP)
SARP provides substance abuse screening, treatment, continuing care and referral for active-duty military personnel. Patients are often seen following referral from the drug and alcohol abuse prevention assistants. SARP also encourages self-referral on a walk-in basis. Immunizations are available at Naval Hospital Lemoore 8 a.m. to 4 p.m. Monday through Friday.
HEALTH BENEFIT ADVISORS (HBA)
Information about your Military Health Plan is available from our Health Benefit Advisors at the main hospital. Our HBAs are available 7:30 a.m. to 4:30 p.m. Monday through Friday.
